A $1,000-a-week-for-life New York Lottery prize tied to a ticket sold on Long Island has officially been claimed, lottery officials announced on Wednesday.
The winning ticket was purchased at Valley Gas & Repair at 385 W. Merrick Road in Valley Stream, and it belongs to Leon Cooke of Springfield Gardens, Queens. Cooke’s lucky ticket matched the first five numbers in the CASH4LIFE drawing on Nov. 3.
That combination earned Cooke the game’s second-tier prize of $1,000 a week for life, which comes with a guaranteed minimum payout of $1 million.
The winning numbers were 09, 10, 16, 38 and 51, with a Cash Ball of 01.
Rather than opting for receiving weekly checks, Cooke chose the cash option, which turned the prize into a one-time payment of $612,240 after required withholdings.
