A Floral Park man claimed a $1,000 A Week For Life second prize this week for matching the first five CASH4LIFE numbers on a drawing this spring, New York Lottery officials announced.
Arturo Juarez’s winning ticket in the May 15 drawing earned him a minimum payout of $1 million. He chose to receive a single lump sum payment of $651,000 after required withholdings.
Juarez’s ticket was purchased at Jays Cards & Gifts, located at 712 Old Bethpage Road in Old Bethpage.
The winning numbers drawn were 8, 14, 25, 36 and 58; the Cash Ball picked was 1. CASH4LIFE drawings are conducted daily at 9 p.m.
